Each participant must have a Sports Physical on file with the Athletic Director. The Physical must have occurred within the last calendar year and have the doctor's stamp to be valid. Download the form below. Each participant must also have a Player Record Card on file. The Player Record Card serves as parental permission to participate and provides emergency contact information.
